The sweep of the book is impressive, from the origins of the idea of complete eradication in the early twentieth century until the present-day campaigns against polio, Guinea worm disease, and now malaria. The author places eradication's story in its many contexts, from imperialism, changing notions of public health, the history of medicine and its technologies, the development of international health agencies such as the World Health Organization, and the impact of the Cold War on the shift of attention to disease in developing countries.
At the center of this narrative is Dr. Fred Lowe Soper (1893–1977), a U.S.-trained doctor who became the arch-eradicationist of his time. His campaigns to eradicate hookworm disease, yaws, yellow fever, malaria, and smallpox are treated in compelling detail, as are the roles of international health agencies such as the Rockefeller Foundation and the World Health Organization.
Throughout the book Stepan draws attention to the way that the ideal of eradication has repeatedly arisen, phoenix-like, from its setbacks. In a powerful conclusion, she uses the example of the current campaign to eradicate Guinea worm disease to argue that, today, under the right circumstances, eradication and primary health care need not be in conflict, as they were in the past, but can form mutually reinforcing policies to improve the health and well-being of populations, especially the poorest and most disease-burdened populations of the world.

Eradication: Ridding the World of Diseases Forever?

The book provides a historic approach to attempts to eradicate certain diseases. It points out the pitfalls of too much confidence and too little scientific information. Students of International Public Health will find the book informative and a good read. Those already in the field can reminisce. I am part of the latter as I met a number of the scientists mentioned in the book. I was at the convention where the two societies of tropical medicine joined to become one. Many of the graduate students and young professionals were told to avoid careers in malaria as it would soon be eradicated. Many of us followed the advice and selected other fields only to spend our careers working on malaria, yellow fever, denque, etc. This adds proof to her observations on eradication. She does a good job identifying the problems and solutions of disease control and/or eradication.
The story is balanced between progress and failure of programs It demonstrates the power of personalities whether right or wrong. I believe in some places she was a little unfair in her views of the Rockefeller staff. Even in failure their accomplishments were considerable and their work laid a foundation for many other groups that have tried to solve problems in the tropics. I noted that the writer did not give much credit to the Carter Center for their work in eradication of diseases. They are usuing a more modern approach and making outstanding progress.
As one who has worked with many of the diseases mentioned, I found it an enjoyable read and a chance to meet old friends as their names appear in the book. A book written in the 1930s called
"An American Doctors Odyssey' introduced me to the adventure of international public health and I think this book has the capacity to do the same for the present seekers of a career in health.
